Transfer a Form to another person

I need to transfer a form I made to a co-worker's account as I will no longer be responsible for the form at work. How do I do that? I have provided the link to the form below.

The form transfer request must be coming to the form owner. Please open the form owner's JotForm account and reply to this support ticket for confirmation:
