I have checked your page and it seems like the issue is fixed, the form seems to load without issues, here is an example about what I can see on my end:
If you continue experiencing the issue, kindly check any adds-on installed on your browser, you may also try using incognito mode and see if that helps, another thing is to temporarily disable the antivirus or any security software so you can see if there is anything causing this issue.
