My form is disabled, but I did not disable it.

My background form says it is disabled, but i did not disable it, it is marked as enabled.  I need help fixing this.

It appears as disabled because your account is currently suspended. It was flagged as phishing because one of your forms collects SSN: https://www.jotform.com/form/201594869422059 

To collect that kind of data from users, you must Signature and Terms & Conditions widgets on your form. While there is already a Signature field, you are still to add a Terms & Conditions to it:

To comply with that, you must authorize us to remove the offending field of your form, then we will reactivate your account. Once you recover access to your account, you must add the missing widget to your form, then you will be able to collect SSNs again.
