Shared forms asked users to create an account

    Hello,


    I did a trial run and sent a Hipaa form link that I created to a family member, and it turned out that the link asks her to create a JotForm account first.  Our patients are not going to like having to create an account just to fill out these health questionnaire forms.  Is there another way around it, or did I not set up the form properly?

    Hi Amy,

    Thank you for contacting Support.

    Please unchoose the following option to avoid that:

    1) Go to PUBLISH.

    2) Then go to SETTINGS.

    3) Choose PUBLIC FORM.

    Hi there,
    Does this mean that anyone, including those who do not receive the URL directly from me, will be able to find the form?

    Yes, when the form is public, anyone who finds the form embedded in your website or it's URL, will be able to submit the form. If you want to make the form password protected, please check this guide: https://www.jotform.com/help/579-How-to-Password-Protect-a-Form
