How do I add JotForm to our practice web site?

    I need to be able to use the forms on a site called Simple Practice.
    How do you migrate your forms to such a platform?

    Are you referring to the Simple Practice EHR software? Do you mean to send the form submitted data to this EHR? If so, there is no direct integration available with this software at the moment. 

    If you are referring to display the form on any of your web pages, JotForms can be embedded on any web page using iframe embed code. You may get the iframe embed code for your JotForm by following the steps in the guide below:
