Misspelled name in value

I have setup a condition and enabled/required field. In a statement if a provider = "name" is misspelled i can't figure out where this is changed. its just a simple form.

I have checked the most recent form you are working on and found a "Provider" field.

Is that the field you are referring to?

Can you tell us what exactly are you trying to accomplish, please?

I see that the field is a dropdown field with "names" as its options.
