How do I get all options to print on my PDF?

I have built a form which consists of a checklist of symptoms.  When I go edit the PDF version, it will only print the options that they checked off - not the whole list.  I need the whole list to print, with an emphasis on the checked off items.  How do I do this?

Thank you for your message, in order to avoid to show all the blank fields, please click on "Hide empty from fields" under the PDF Editor.
