Appointment Widget: part of it disappears

hi I have used an appointment part on a form but after two submissions the appointment park of the form disappears 

please see below first picture is right 

then after two customers submit the form the picture below is what the next person sees 

can you please let me know why this is happening

Please note that we had an issue with the widget, but it got fixed today by our developers.

Could you please check if this issue still persists on your end?
