I have checked your form and found the Sender Email is not setup to email@example.com as it has to be. That might be the cause of your issue, as that is a very important detail.

I'm suddenly not receiving emails today when before I had no problem. My sender email was always set to the email address of the person filling out the form and I had no problem before. Why suddenly must we change to the noreply address?
