Question regarding online forms

We were referred to you by Arcadia IT that takes care of our website. We are an Endocrinology practice looking for a way to have patients fill out forms, sign consents, etc in a secure fashion online. Presently we have our forms on our website as fillable pdfs. After the patient completes and signs they are emailed to us. We know that the HIPPA regulations are going to tighten up again at some point so we need to find an alternative method to do this. We have also run into problems with the forms not holding information in the format that we have them now.

Does your service provide forms that we would integrate into our website or do you have an option for the patients to connect directly to you to fill out these forms?
