Creating a new form

Hello! I am a new customer. I am figuring things out pretty well but I need some advice. I am an acupuncturist. I am adding online consultations to my services. I have figured this out pretty well in terms of creating an Online Patient form. This is where I need some advice:

When I complete a consultation, I fill out a form that includes treatment recommendations, treatment fees, and payment options. So...this is a form that I fill out and give to the patient. What is the best way to create a fillable form (THAT I FILL OUT) and can email to the patient immediately after the consultation?

It is possible to create a fillable form by using URL parameters.

You may use the populate app:https://prepopulate.jotform.io/ to easily populate your form and use the URL link and send it to your respondents.

To use the app, you would have to fill your form fields and then click generate URL button if you are done .
