Repeating fields

    We would like to have select fields be repeated at the top of each page without the user having to re-type the information.  An example would be the name and age (of the patient).  Right now, this information is only on the first page because we didn't want to ask the user to complete it multiple times; however, our physicians don't always use the first page which is more for administrative purposes.  Is there a way to take the first name/age field and make it auto-complete at the top of each new page?

    You can the below fields to the other pages. In this way, you do not have to type the name or age again. You will only need to type it once.
