My name is Randall Mills, CEO of Healthcare Now, Inc.  We recently changed our website 24hourphysicians.com and no longer use Jotform.   Our IT manager Alan Dip may have signed up for Jotform using paypal account which was charged $39 on May 24. Please discontinue this subscription. Thanks.
