Notes on submission

I use your forms to gather information from tattoo clients as well as the waiver forms they need to sign. It works well but I would like to know if there is a way for me to add notes to a form submission once I have received it. For instance, I would like to be able to make not of the clients temperature as well as the price of the tattoo. Thank you.
