Leads not getting created in Salesforce

Good morning,

We are currently using two forms for the same purpose. While one form works just fine, we are having issues with the second form.

When I try to submit a lead it goes through and gets created in Salesforce just fine, but all leads that submitted the form are not coming through.

Please help!

Could you please confirm if the form is working wen is submitting with the direct link https://jotform.com/201547024112137, if the lead is generated using the direct link please go to your app publish settings and copy again the source code to your website.
