Radio buttons for yes or no questions

    Hi!  I made a form with JotForm and it looks very nice except the radio buttons for yes or no and consent questions don't appear either in PDF when I'm filling it out or on the printed version.


    Any advice?

    Thank you for your message.

    After checking your form I can see that under the PDF editor you have the checkboxes already added:

    Could you please confirm if the PDF is not showing the checkboxes? also please provide us with one PDF file, you can use Google Drive or Dropbox to share the file.
