Need Jotform to complete W9/Become a vendor

Our agency (Cuyahoga County Board of Developmental Disabilities) needs to make Jotforms a vendor so we can pay for this service from an invoice. We will be able to pay a full year at a time (annual invoice). Could you please complete the attached/uploaded W-9 form and email it to me? Thank you.
