I've created this custom form, and I'd like to integerate it with Jotform.
Meaning, I want to use the the form I've created and integertate the field so that they match exactly with the actuall form.
The problem I'm having is that how do I go about change the ID/Name of the field so that they match with the actual jot form.
Also, how would these setting change, if they do?

Please describe to us how you meant by "integrate"? From what I understand, this is not a JotForm form
You can actually create a similar form in JotForm and embed it in your website.
And you can Send Post data form Jotform using this guide
