Add multiple columns AND rows of text boxes to fill by patient?

Hi

I would like to add ad question that ask - what medications are you taking, please indicate Name, Dose, Frequency and condition treated. How do i add text boxes on same line, 

NAME        DOSE            FREQUENCY          CONDITION
