I can't seem to get my forms downloaded as a PDF

Good morning,

Our swim team operates as an affiliate of a homeowner's association and they've asked us to collect waivers from all participants. I've sucessfully downloaded from this form before when we held our CPR class but I can't get the remaining responses to download. A message pops up that says I'll get an email but I never do. I just need PDFs with the verbiage and signatures for all submissions. Can you help me figure out what I'm doing wrong? I've checked my spam filter and it's not there.
