Not seeing job request

Hello,

I have a question - One of the facilities I staff stated they entered a job request using jotform 2 days ago but the request isn't coming through on my end. I cannot see the job request like before. PLEASE HELP! I used to have a form specifically for that facility but it disappeared.

I checked your account and there's no form. Could you please share the form link so we can check it further?

Also, I checked your IP address and there are multiple accounts associated with it.

Is the form embedded on a website? If yes, please share the link so we can test it too.
