Embedding form in Shopify

When we purchased your application, Shopify was listed as an integrated solution and was listed on your Integrations page.  Today it is no longer listed there.

It is URGENT that I know if your solution is still integrated with Shopify.  We are building out forms for our store TODAY. 

Also - if there is a customer service phone number please post it here with your response.  I can't find it anywhere.  thank you! 

This should be possible to embed forms in Shopify by adding the form embed code to the CUSTOM HTML element in Shopify.

We do not offer support over the phone, but we will be happy to help you over this support system.

But it doesn't integrate with the cart - we need it to add to the cart as part of the sale.  Am I missing something?  Isn't this the only reason people add your product to shopify - i.e. so customers can check out for products that call for file uploads?   

It seems that you can include a form in regular pages, product pages, order status pages, and include a form link in emails.

According to this Shopify community thread, embedding into the checkout code is not supported. However, I am not a Shopify specialist, it is recommended to check this with Shopify experts.
