Product question - Application form configuration

I need a program that I can have someone fill out an application, see their status on that application and then be able to build a client profile if their application is accepted. It would also be nice if the could get automatic emails for when they complete the application or get accepted or denied. Please let me know if this is something you guys do. 

JotForm is usually used to create web forms and collect data. You can use our form builder to create custom web forms. The automatic emails are supported.
