Inserting Text in an agreement

Is it possible to insert text box in an agreement such that the recipient can fill in the missing information on the fly.

No, am not referring to Terms and Conditions. I mean if I have a text that is similar to the below where the respondent needs to fill in the information on the dashed line. Is there a way to insert a text box so this is possible?

Unfortunately, this widget does not have the feature, I can make a request on your behalf, however, I cannot guarantee an ETA of when it will be developed and implemented.
