Need a 2 page legal waiver to email our customers

We need to be able to email our customers a 2 Paige COVID19 waiver that they can read, sign and generate and email back to us to store.

Could you elaborate on what kind of fields would be included in the waiver?

Our builder allows you to add basic fields for the client to fill with their personal information. You can use text fields to add information for the user to read. 

With regards to signatures you can take a look at one of our signature widgets for you to use which would allow the user to sign the waiver:
