Forms that have been previously set up and working OK are now showing an error over the selection criteria.
The error reads:We cannot complete your request.
All settings are correct, our account is well below the form limit.
Can you please advise how to fix this issue as it is required for business appointments.

I am sorry for the trouble caused to you. I could not test your form as you seems to have disabled it.
When I checked your form in editor, it seems to display the widgets correctly. Please check the screenshot below:
I would suggest you to please test your form again and get back to us if the issue persists.
