Is it possible to create a survey without participant data as email?

You are able to choose what fields you wish to add to your form. If you don't wish to send Autoresponder emails to your users when they submit the form, you don't need to ask them for an email. Additionally, your users don't need to have a JotForm account to fill the form. You can send them a direct link of the form, and they are able to fill it without creating an account. 

If you are referring to creating a form without providing an email, you are able to do so, however, you won't be able to see and manage submitted data.
