I created this form under a guest account:
but now that I created an account, I can't access this form. Can you help? I've changed computers recently, could that have anything to do with it?

I don't think that changing the computer caused the problem. According to my research, if you go into your account settings and change the email to the one previously used for the guest account, the missing form should be transferred to your Free account.
Your account settings can be accessed via the "Account Settings" button found near the bottom right of the "My Forms" page.

Thanks. The problem is that I can't even find the form anymore, It doesn't appear in My Forms, and if I log out and come back to JotForm, it tells me I have no forms created. I access the form with the url I put above, and I do receive submissions, so I know it's out there...

Yes but you don't need to find the form. If you change the email address presently in your Free account to the one you used for your Guest account (hint: misterch...[at]gmail.com), the missing form will be automatically imported into your Free account. (I have never tried this because I have never had a Guest account but that is the solution I have seen mentioned in some threads of this forum.)
