I want to share a form I created with a select group of people.

I am a doctor and I created a form specifically for concussion and I want to share it with people in my group so they can upload to their account and send it to their patients. That wya they can receive the responses directly. share as template looks like it just goes into the whole mix. is there a way to do what i want? if so how?
