    Is it possible to mask or remove my account name, and replace it with a unique ID in the published URL to a form?

    I assume you're using the custom form URL which includes your account username, the other alternative is to use the default form URL which only will include the form ID, this guide will help you to reset the URL: https://www.jotform.com/help/261-How-to-Create-a-Custom-URL-of-a-Form 

    It's also possible to use a custom domain on your forms, this feature is however only available on the enterprise level, here is a guide with more details about it: https://www.jotform.com/help/144-How-to-Use-Custom-Domains-with-JotForm-Enterprise

    Does this mean I copy the unique form ID shown in the URL into the custom portion, or it generates another ID?

    What happens to the original data collected with the old URL?

    You need to copy the entire form URL as it's given by the publish wizard, but that will have the form ID which is unique and does not change. 

    The submissions are not affected by this change either.
