Why is my account disabled

Hello, my name is Steve Hunter and am part of a non-profit organization called McDowell Trails Association.  Our organization has been using 1 JotForm on our web site for new memberships.  recently, I created a new Donations form to be added, but noticed now the account is disabled.

Please advise on enabling the account

I included some screen shots of the web site where the error occurs that the form is disabled.

You can see this error by going to mcdowelltrails.org then “Join and Donate” and then “Join MTA”
