Form conversion from original PDF

We have a form that we use and want to know if JotForm offers an option for migrating our forms. When we upload our form and try to manipulate it we don't get a desirable result. I have attached an example, no PII or PHI is on the example.

I checked on your form and I see you have not re-positioned yet all the form fields into the original PDF document.

You need to drag and drop the fields and position them in the appropriate fields/questions in the original PDF document.
