Route Form for Approvals

Hi, I am wondering if anyone has any suggestions for me. I need to be able to route a form for approval but the form has to be routed to certain people depending on the response. So for example if I have a request form submitted, I need the form to be approved by the specific department head based on the department indicated in the form response.
