This form is supposed to foward to our email and has worked fine until the last few days. Very important to resolve asap. It shows up in the jotform "inbox" but does not forward to the e-mail.

Very sorry for the inconvenience caused. Can you please provide us the Form URL for which you are getting this issue? So that we can check our mail logs and investigate further.
Sorry for the inconvenience caused. You mail ID came into our Bounce list for some reason. Now I removed your mail ID from our Bounce list. Can you please check now.

The JotForm Support Team Deeply apologizes for the inconveniences that may have caused.
This issue is due to a technical problem in our servers that is affecting our email notification system. Our team of developpers is already working hard to solve this issue asap. That's why many of you are having problems to recieve your notifications.
Due to that issue your email was found in the bounce list:
Result: firstname.lastname@example.org was IN the bounce list
Reason(s) : Amazon blacklisted.
I have already removed it from there. But to prevent that happens again, follow these instructions:
Change the Sender Name to the to the Name field of your form, it can be Who are you? in your case.
And setup the Reply-to to the email field of your form. That should look like the example below:
