Form Heading Title and save name

Hi

I have completed several forms but the form is taking data from the first data field such as a persons name or an items name and is using that at the forms title and save name i.e. When I create the form Safety Audit and have Safety audit in the title but when i submit the form it has the name of the person as the title and save name.  Is there something I can do to stop this it is very annoying.  Thank you

I am not sure if I have understood your question correctly. Do you mean to say that submission email shows form name and the field name?

That is because you have set the form name and auditor Name in the subject line of submission email. You can type any text and/or select any field data to be displayed in subject line of email. Please check the screenshot below:
