Form won't submit - Please Wait

My Commitment to Erie Submission form is stuck on "PLEASE WAIT" while trying to submit an entry.

How do we fix that? 

I cloned your form and tested it. It seems that this happens because of the Upload field.

Can you please try to add a new Upload field on the form and hide the old one? You can also delete the old Upload field, but please note that this may also delete some submission data.
