    Hello,

I generated a survey/questionnaire form about a month or two ago and saved a link to it.  However, when I went back into my JotForm account yesterday, I see it isn't listed there.  I'd like to edit it.  Do you know why I am not able to access it through "my forms" or why I am not able to edit it when I access it via the link I saved?  Here's the link to the form:  https://www.jotform.com/form/201276225216144.

I'd really like to use that form to edit it by changing the background color, adding my logo, and removing some of the item choices I have.  I have since used the pdf I saved of that form and recreated it by moving it back into an online form, but now the items chosen no longer add together (per response) to give an overall score.  I even looked at recreating it, but I can't seem to find the template I used at that time. Any help you can provide would be wonderful!!!!

    Thank you so much,

    Tracy
