    Hello,

I have an account under the email address christina@syncaccounting.com that was suspended.

Side note - I would like to reactivate it and when I initially tried to use that address on this form it wouldn't let me.  What was frustrating is that I couldn't use my account because it's suspended even though I have no clue how my account was "phishing", then it said I had to use this form and then the form said I had to log into my account before it would post my question so I had to re-post this and try with a different email address. It just doesn't seem logical from a customer support perspective.

    Cheers,

    Christina

Hi, thank you for reaching out to us.

As per checked on your account, your form 200767014225245 has been disabled for collecting Social Insurance Number which violates our Term of Use

Online Form Builders, including Jotform, are sometimes being used for identity theft and we go to great lengths to prevent this. That is why collecting these types of data is considered as phishing activity, as stated in our terms of use.

In order to reactivate your account, we need your permission to delete the offending fields.

We look forward to your response.

I just wanted to follow up. I run an accounting firm and would love to use your service as an intake form but SIN's are vital for us. Just a thought that you may want to reconsider your policy or just make it very clear that accountants/tax preparers are not your target market.

    Hi,

Online form builders, including Jotform, have been used for identity theft in the past. We go to great lengths to prevent this. That is why collecting these types of data is considered as phishing activity, as stated in our terms of use.

If your purpose is not to collect SIN from the general public, but from your own customers; then all this will surely make your forms not to be considered as guilty violating our Terms of Use.  However, you need to comply with two things:

    1. Add the Terms & Conditions Widget (or the Short Scrollable Terms Widget) for your users to read and agree.

    2. Add a Signature Widget so your users can sign and agree to provide such sensitive information.

Let us know if you have any other questions.
