I have cloned your form and was able to submit it without any errors. What kind of error do you get when you try to submit your form?

Thank you for following up. I was able to fix the problem. I had to use a
different feature. The specific feature in this form that was of error was
for email address only. I changed it to long text box so that people can
write more information in it. It is working fine now thank you
