    Hello, my form is disabled, there are no fields asking for sensitive information, the fields ask for the following:


    Name

    Ingame Nickname

    Phone number


    Kindly advise

    The account was suspended due to this form: 201683383722052 for offering a PUBG tournament and using a copyright logo.

    In order to reactivate your account, we need more details about your Organization/Company. Kindly provide more details on the following:

    1. The URL where your form is used.
    2. Your website or proof of affiliation to a legitimate institution.

    We also need you to change your registered email to a corporate one to avoid getting flagged again by our phishing detector.
