    Your account was suspended due to the form 201676111527248 for collecting login credentials.

    Online Form Builders, including Jotform, are sometimes being used for identity theft and we go to great lengths to prevent this. That is why collecting these types of data is considered as phishing activity, as stated in our terms of use.

    We will only lift the suspension on your account if you agree with our terms and allow us to delete the offending fields mentioned above.

    Hello there,


    Thank you for the response. I think the fields have been removed. It happened when I tried to import form from a webpage, where it required login info. I agree to the terms and please delete the offending fields mentioned.

    Thank you very much!

    One follow-up question: we're a visa and immigration tech start-up in Canada. Due to the nature of our business, we will be collecting personal information such as name, email, phone number, address, date of birth, age, education, marital status, driver's license number and etc.

    According to Term of Use, there's no exact definition of sensitive personal information. Are we able to collect the information mentioned?

    We just want to confirm this issue before moving forward with jotform. We're using jotform for data collection. Any payment or login information will be collected and stored via our own server / website.

    Hi,

    Online form builders, including Jotform, have been used for identity theft in the past. We go to great lengths to prevent this. You can collect personal information from your users except for credit card information and login credentials, SNN, Driver's license and other sensitive data. Collecting these sensitive data is considered as phishing activity, as stated in our terms of use.

    If your purpose is not to collect the data from the general public, but from your own customers; then all this will surely make your forms not to be considered as guilty violating our Terms of Use.  However, you need to comply with two things:

    1. Add the Terms & Conditions Widget (or the Short Scrollable Terms Widget) for your users to read and agree.

    2. Add a Signature Widget so your users can sign and agree to provide such sensitive information.
