Cannot connect checkbox field with PDF

I'm working on converting a PDF form into a fillable one, but I'm having an issue with the checkbox option when trying to place it on the pdf document. It is not allowing me to drag out the field as a box like the previous 3 lines above within a table. I've included a screen shot of what I'm experiencing. If someone could assist me with resolving this, I would appreciate it.

I've cloned your form and could solve the issue by deleting the field on the fillable PDF and adding it back again.
