PDF form converter is changing the form

    The form uploaded changed coding when sumbitted. How can I get the coding not to change?

    Hello,

    I just added the form back on. It is labeled Health Questionnaire and you will see a little half way down the wording is all messed up in weird scripture

    Hi there,

    Thanks for your response. 

    I apologize for the inconvenience. I have tested your form having the weird scripture. This might happen during the PDF conversion but, you can still edit the wording. Please follow these steps to edit the scripture:

    1) Goto your form edit option

    2) Double click on the scripture you want to edit

    3) Once done editing click on the save icon. 

    Now preview your form to see the new changes.

    I did that but down at the bottom where the "Yes" are there needs to be yes and no with boxes next to it to be able to check the box and they have disappeared. Can you see the original PDF?

    You can edit the form as much as you like and then map the fields to the PDF. I've cloned your form and made a demonstration screencast. See below:
