Uploaded a PDF, but now having problems identifying headers, where check boxes belong and do not belong (incorrectly came in with the upload), sectioning the form, etc.

If the PDF you uploaded is not fillable, it won't be recognized by Jotform's PDF converter. You can convert your PDF to fillable for free from here and re-upload it to Jotform again.
You can also share the PDF you are trying to import so we can check. Looking forward for your response.
