If the PDF you uploaded is not fillable, it won't be recognized by Jotform's PDF converter. You can convert your PDF to fillable for free from here and re-upload it to Jotform again.

You can also share the PDF you are trying to import so we can check. Looking forward for your response.

We have received your email regarding your concern. We appreciate you providing us the PDF form you are trying to convert to an online form. I understand that some of the fields are not formatted correctly on your end when you converted your PDF form.

I have converted the PDF form you shared on my end and made some adjustments to its format. Here's the link to that form: https://form.jotform.com/201777385229060

I would suggest that you create a cloned version of that form in your account so you can inspect it better. Reference Guide: How-to-Clone-an-Existing-Form-from-a-URL

Feel free to let us know if you still need anything else to be adjusted in the converted PDF form and we will be glad to help you.
