Account Suspended due to collection of Credit Card details

    I'm creating a digital version of a credit application for my company to go paperless...i actually don't need the data stored in your website but merely the fact that the user can produce a pdf file at the end of the form and send it to our credit department when looking for financing options while in the sales process, if google forms could do it i would have done it already there.

    The current method its taking pictures of such filled credit application and sending them through whatsapp because is encrypted.

    I have been testing the form and accessing from my phone and computer but have not distributed to anyone else or put actual sensitive information in it at all.

    Good that you have a phishing prevention system but should have warning messages when creating fields or at least when you OCR the PDF...the form is called CREDIT APPLICATION and have been sitting in there for quite long now, there is no scam intention here.

    Now my account it's suspended.

    Your account is currently suspended as some of your forms were collecting Credit Card details.

    Collecting credit card details using regular text fields is not allowed here in JotForm. Please check our terms of use.

    If you need to collect payments from your users, please use one of our payment processors. Here's a guide: Setting-Up-Your-First-Order-Form.

    You may also capture the card details and charge the user at a later date. Here's a guide: How-to-Enable-Payment-Authorization.

    Was that it...a field that says "Credit Card type"?

    Its not even a credit card number field or a "put the 3 numbers from the back" field.

    That only serves the purpose of stating that the customer has some sort of credit when we offer in home financing.

    Ok, im wondering if at some point i can continue my project making changes?

    Please note that CC details including types and expiration dates are still not allowed. It seems you also have added SSN and DL fields in form as well. 

    You may continue to collect that information, but it is required to add an agreement displayed on the form and a signature, widget fields. You may like to take a look at the following guide on how to add widgets to your forms:

    I have deleted the credit card related fields and activated your account for now. Please ensure that you make the changes suggested above to avoid automatic suspension of account again.

    I removed the remaining cc boxes and added the license agreement checkbox at the end of the paragraph...and even created some conditions to ensure they check the agreement.

    But again I don't have acces to the form and I'm about to start the pilot tryouts for feedback, please help.

    I apologize for the inconvenience. I believe these 3 fields have triggered the suspension that were still present on the form:

    - expiration date: as my colleague said, this is unfortunately not allowed

    - 2x "Take a photo of the ID..." fields: please note that it is only allowed to collect ID numbers, with the 2 mandatory widgets, but we do not allow the collection of photos/copies of IDs

    I have removed these fields and reactivated your account.

    I would suggest you read our Terms of Use carefully to be able to comply with them and avoid future suspensions.

    Thank you, the form is still disabled even with the corrections done, do I need to wait on something else or is there anything else to take away or maybe change?
