Mengapa formulir yang saya buat tidak bisa terbuka? Lalu mengapa akun saya ditangguhkan?

Selamat Pagi Min,

Kenapa akun kami kembali ditangguhkan dan formulir PPDB di sekolah kami tidak dapat ditemukan? Kami sudah mengikuti anjuran terkait pemakaian atau penggunaan formulir. Jika sebelumnya permasalahan kami hanya pada mengunduh atau memberikan akta lahir, lalu saat ini permasalaannya apa?

I did check your account and found that it is currently suspended because of the "ID Upload" field you have added in form "Formulir PPDB SMP AL IHSAN JAKARTA". Please check the screenshot below:

You may collect identification numbers only, but you must add an agreement displayed on the form and a signature, widget fields.

Saya hanya sekadar ingin menanyakan, mengapa kami tidak bisa menambahkan pengumpulan KK atau Akte dalam formulir yang kami buat? Padahal, data tersebut sangat kami butuhkan untuk diserahkan kepada dinas untuk pendataan siswa baru.

You can actually collect valid ID and document uploads to your form. If you are collecting this information from your students and not from the general public, we can make an exemption. However, it needs to be protected by these two mandatory widgets: 

1. Add the Terms & Conditions Widget(or the Short Scrollable Terms Widget) for your users to read and agree.

2. Add a Signature Widget so your users can sign and agree to provide such sensitive information.

Also, we recommend that you use a corporate domain for your email instead of a free one that you're currently using.
