I please need urgent help with my form - everything has gone haywire after changing themes

i please need help again, i changed the theme on my form due to accessibility issues, now colours are out , my matrix is out of line.

o don't have proper borders, other colours are showing up between my matrix?

i need to share this form in the morning before 8 am,  please please help to make it presentable?
