Account Locked - Please Unlock

    Can you please advise as to why I'm locked out. This is the 3rd time my account has been locked. I have not made any changes to the forms so I'm not sure why it's locked. The E-Signature & Terms & Conditions were not altered. Please advise ASAP.

    Hi Lori,

    I apologize for the inconvenience. Your account got suspended because of the email address you are using with it.

    "farmersagency.com" is not a valid domain. The logo you are using requires a valid domain.

    Please send us proof that you are allowed to use this logo:

    1. The URL where your form is used.

    2. Your website of proof of affiliation to a legitimate institution.

    We also need you to change your registered email to a corporate one to avoid getting flagged again by our phishing detector.

    Send these proofs to reviewers@jotform.com with Thread #2399870 as your subject. We will let you know about the status of your account after a thorough review of your proofs.

    My apologies. My first submission I stated my personal email address but the account is setup with my work email. My husband is a district manager for Farmers Insurance. My email address is a valid email address: lori.rmerritt@farmersagency.com. It was provided by the company, Farmers Insurance.

    I can remove the logo or do you have to do it? It hasn't been an issue using it until now. Not sure why it suddenly became one. Please advise. Are there any other issues that would lock my account? This is becoming a big hassle....I love Jotform, but don't want to keep dealing with my account being locked.

    Please sent the proofs my colleague asked for to reviewers@jotform.com with Thread #2399870 as your subject or please confirm that we can remove the logo.
