I am getting a message that my account has been suspended for phishing.
What do i need to do to unlock it?

Hello Christopher,
Your account was suspended due to the credit card fields in one of your forms and user credential fields in the other one.
We do not allow forms like this as it is considered phishing which is against our Terms of Use (https://www.jotform.com/terms/).
I have deleted the fields and lifted the suspension.
If you need to collect payment at a later date, you can do it with our payment integrations and by enabling Payment Authorization.
