Suspended Account

    There was a mistake done on the account related to the terms of service. The form was supposed to be used as a truck driver application which required a social security number, since this was against the terms of service the account is now suspended. We were wondering if we can change the form so it doesn't include anything that breaches the terms of service because we would like to use this account for job applications.     I would like to apologize for the inconvenience that you are experiencing.

    It seems that your account has been suspended for requesting Driver license and Social Security numbers in your Application For Employment form.

    JotFrom does not allow the gathering of private information such as Driver's License, Social Security, Passport, or other government-issued documents without the proper security measures.

    To have your account re-activated you will need to once again agree to our Terms of Use by allowing us to delete the offending fields or the entire form. We will wait for your response to proceed.

    For future reference, you will need to do the following in your form in order to correctly request this type of private information.

    1) Insert a Terms & Conditions widget into your form with your Terms of Use

    2) Insert an E-Signature widget into your form so your respondents can acknowledge that they are providing sensitive information

    3) Change the email address on your profile to one that shows your corporate domain.

    Please do get back to us so that we can make the necessary adjustments to your form in order to unsuspend your account. We look forward to your response.

    We agree to the terms of service and for you to make the necessary changes to the form. On our front, we will follow the instructions highlighted here. Once we log in the email address will be changed to one with our corporate domain. Thank you for your time.

    I have removed the offending fields from the form and reactivated the account. You may now log in again to the account.

    You can add the deleted fields back but please ensure to comply with the mentioned requirements as well. This is to prevent your account from being suspended again in the future.
